How to fill any State Bank of India form

  1. Download the form from the official bank website linked below, or pick it up at any branch.
  2. Fill every field in block letters with black or blue ink; do not leave mandatory boxes blank.
  3. Enter your account number and registered mobile number exactly as the bank has them on record.
  4. Attach self-attested copies of the required documents listed for that form.
  5. Sign in the same style as your account signature — mismatched signatures are the most common reason forms are rejected.
  6. Submit the form at your home branch (or the nearest branch) and keep the acknowledgement slip.
