Account Opening Form
Purpose: Open a new savings or current account.
Who needs it: Anyone starting a new banking relationship with the bank.
Documents required: Photo ID (Aadhaar/PAN/passport), address proof, passport photos, and initial deposit.
KYC Update Form
Purpose: Refresh your Know Your Customer details (ID, address, contact).
Who needs it: Existing customers whose KYC is due for periodic re-verification.
Documents required: Updated ID and address proof, recent photograph, and account number.
Cheque Book Request Form
Purpose: Request a new cheque book for your account.
Who needs it: Account holders who have run out of cheque leaves or need a fresh book.
Documents required: Account number, registered signature, and (sometimes) the requisition slip from your old book.
Address Change Form
Purpose: Update the mailing or permanent address on your account.
Who needs it: Customers who have relocated or whose address proof has changed.
Documents required: New address proof (utility bill, Aadhaar, rent agreement) and account/customer ID.
Nomination Form (DA-1 / Form 'A')
Purpose: Add or change a nominee for your account or deposit.
Who needs it: Any account or FD holder who wants to nominate a beneficiary.
Documents required: Nominee's name, address, relationship and date of birth, plus your account details.
NEFT / RTGS Form
Purpose: Authorise an offline NEFT or RTGS transfer at the branch.
Who needs it: Customers making a fund transfer in person rather than online.
Documents required: Beneficiary name, account number, beneficiary bank IFSC code, amount, and your account details.
Fixed Deposit Form
Purpose: Open a fixed or recurring deposit.
Who needs it: Customers placing a lump sum (FD) or monthly amount (RD) for a fixed term.
Documents required: Account number, deposit amount, tenure, payout/renewal instructions, and nominee details.
How to fill any ICICI Bank form
- Download the form from the official bank website linked below, or pick it up at any branch.
- Fill every field in block letters with black or blue ink; do not leave mandatory boxes blank.
- Enter your account number and registered mobile number exactly as the bank has them on record.
- Attach self-attested copies of the required documents listed for that form.
- Sign in the same style as your account signature — mismatched signatures are the most common reason forms are rejected.
- Submit the form at your home branch (or the nearest branch) and keep the acknowledgement slip.
